Open app.formalert.in in Safari (not Chrome).

Tap the Share button Share at the bottom of the screen.

Scroll down and tap "Add to Home Screen".

Tap "Add" in the top-right corner.

Must use Safari — Chrome on iOS does not support Add to Home Screen.

Android — Chrome

Open app.formalert.in in Chrome.

Tap the three-dot menu ⋮ in the top-right corner.

Tap "Install app" or "Add to Home Screen".

Tap "Install" to confirm.

Desktop — Chrome / Edge

Look for the install icon ⊕ in the browser address bar.

Click it and follow the prompts to install.
