In one paragraph
FormAlert is a clinic management system for doctors in independent private practice and small clinics in India. It runs the appointment side of the practice — online, QR code and WhatsApp booking, walk-ins and phone bookings in one queue, automated reminders and Google review requests — in one tool. Patients book from a web page, by scanning a QR code at the clinic, or by messaging the clinic on WhatsApp — no app to install and no form to print. The clinic confirms the appointment and marks the visit complete; FormAlert sends the confirmation the moment it is confirmed, reminds the patient 24 hours and 2 hours before the visit on the Business plan, and asks for a Google review afterwards, without anyone at the clinic typing a message. Starter is ₹999 a month and carries one doctor and one login; Business is ₹1,999 a month and adds the reminders, more than one doctor with a login each for them and the front desk, and the clinic's own WhatsApp Business number. There is no chatbot: every automatic message is about an appointment, and a person answers anything a patient actually asks.

What it does
Everything else in FormAlert exists to make these three happen without anyone remembering to do them.
Mark a visit complete and the Google review request goes out on WhatsApp by itself. More reviews means your clinic ranks higher on Google, which is where new patients find you. Nobody on your staff has to ask anyone.
Both plans
A WhatsApp reminder 24 hours before the visit, and another 2 hours before. The patient turns up, or tells you they can't in time for you to give the slot to someone else.
Business plan
Collect a deposit or the full fee by UPI, card or net banking while the patient is booking, through your own Razorpay account. The money goes straight to your bank — we never hold it.
Both plans
Who it's for
FormAlert manages the practice, not the medicine. There are no clinical notes, no prescriptions and no medical history — it runs the schedule, the money and the reputation, next to whatever records you already keep.
You want new patients to find you on Google and book without calling. Reviews are the lever, and asking for them by hand never happens on a busy day.

Your appointments live in a notebook, a phone and someone's memory. You want one list that everyone at the front desk is looking at.

Proof
Aries Obgyn Clinic in Guwahati turned on automatic review requests and stopped asking patients in person. 79 by the ninetieth day, 87 six months in, at 100–150 patients a month. Their profile is public — go and look at it.

The manual scheduling and admin chaos was a nightmare. Now patients book online, reminders go out automatically, and we get 15–20 new Google reviews every month without lifting a finger.

Pricing
The difference is whose name is on the message. On Starter it comes from FormAlert's verified number. On Business it comes from your clinic's own WhatsApp number, and you get the reminders.
Business
₹1,999 /month
For clinics with more than one doctor, or more than one person running the day.
We set it up with you. Most doctors have never touched the WhatsApp Business API, so we get on a call and go through Meta's signup together — after that the number and the account are yours.
Starter
₹999 /month
For getting started today. Messages go out from FormAlert's verified number.
Set it up yourself in about 20 minutes. No card needed.
How messages are billed. On Starter each WhatsApp message costs ₹1, and the plan includes a ₹500 wallet every month — about 250 visits. If the wallet runs out, bookings and your dashboard keep working; only the WhatsApp messages pause until you top up from the dashboard. On Business there is no wallet, because the messages go from your own number and Meta bills you directly. Both prices are before GST.
Common questions
One. The booking, the reminders and the review request all come off the same appointment. A patient books online, by QR code or on WhatsApp; the confirmation goes out when you accept it; the reminders go out before the visit on the Business plan; the Google review request goes out once you mark the visit complete. There is nothing to connect up and nothing to switch between.
Sign up and add your clinic details — working days, how long a visit takes, and what you offer. It takes about 20 minutes. You get 14 days free, and during the trial you can send review requests and see for yourself whether patients actually leave reviews before you pay anything.
₹999 a month. Appointment confirmations to patients, an alert to you on every booking, and a Google review request after every completed visit. Messages go from FormAlert's verified number. Each WhatsApp message costs ₹1 and the plan includes a ₹500 wallet each month, which covers roughly 250 visits.
₹1,999 a month. Everything in Starter, plus reminders 24 hours and 2 hours before each appointment, logins for your team, and your clinic's own WhatsApp Business API number and message templates. Connecting it is one Meta popup in your settings and we go through it with you on a call. Most clinics pick Business because they want patients to see their own clinic name on the message instead of a shared number.
Business, if you want your own WhatsApp number, the reminders, and logins for your front desk and your other doctors — that is what most clinics are here for. Start on Starter if you are running the clinic on your own and want to be live today; you can move to Business whenever you like.
No, and you should not have to. Connecting your number is one Meta popup inside your FormAlert settings, but almost nobody has done it before, so we book a call and go through it with you — the popup, the templates, the business verification if you need it. Once it is connected, the number and the Meta account are registered to your clinic and you run them however you like. If you already use the WhatsApp Business app, we can link that number instead and you can send the same day.
Bookings, payments and your dashboard keep working normally. Only outbound WhatsApp messages pause until you top up, which takes a minute from your dashboard. On Business there is no wallet — Meta bills your clinic directly.
No. Your staff types those in, and that part does not go away. What goes away is the work after: no checking the book, no calling back to confirm, no remembering the reminder, no asking for the review.
No. There is no chatbot and no AI replies. What is automatic is every message tied to an appointment — the confirmation, the reminders and the review request. If a patient asks a real question, someone on your team answers it.
No. WhatsApp and email only. In India, SMS gets blocked or ignored too often for us to charge you for it. If you must have SMS, FormAlert is not the right fit, and we will tell you that.
Yes, on both plans. Take a deposit or the full fee by Razorpay and UPI at the time of booking. It goes to your own account. Video visits run on Google Meet.
Cancel from your dashboard. No phone call needed. Your patient data is yours and you can export it any time.
Sign up, add your clinic details, and send a few review requests. You will know whether it works before you have paid anything. No card needed to start.
